The assessment
Make the visit about your problem.
Bring the question that led you to search, not just the name sermorelin. A useful conversation gives you room to explain what has changed and ask what the plan means.
Bring the details you want considered
What has changed
Bring or write down
Describe when the problem happens, what it feels like and how it affects your day.
Ask during the visit
What possible causes are being considered?
Your care so far
Bring or write down
Gather the medicines, supplements, past results and care notes you want the person treating you to review.
Ask during the visit
Which parts of my history matter to this decision?
Your goal
Bring or write down
Put the change you hope for into ordinary words.
Ask during the visit
How would we judge whether the plan helped with that goal?
Use the notes as prompts. There is no need to make your account sound medical.

Care standards
An assessment is more than a treatment menu
The Federation of State Medical Boards is an association of licensing boards. As of September 2026, it rejects a prescription based only on a fixed form unable to gather the needed history.
It also advises giving patients the chance to ask their own follow-up questions. This advice helps boards write their rules. As of September 2026, the person treating you must follow your state's rules; the advice itself is not a law.
A form reviewed by a person treating you who can ask more questions is different from a form that leads straight to a prescription.
Read the medical-board prescribing guidance
Ask what a test would change
The sermorelin studies used blood tests to check growth hormone and blood sugar. Growth hormone is a substance that carries messages within the body. These were research tests, not a list of tests every patient needs.
Ask why each proposed test is needed, who will read it and which decision depends on it. Ask how a result will be explained to you.
Medical-board guidance says a person treating you remotely must arrange needed testing or refer for further assessment when the care requires it.
A question to keep
What would this test tell us, and what would change after the result?
